- You will be seen by a certified practitioner in the MYK system that rebalances your posture/nervous system to improve mobility and pain.
- No double booking or passing your care on to an assistant, tech, or another PT.
- Likely be fewer visits to achieve your goals in a shorter time.
- Your time will be spent learning specific exercises that are cost-effective and easy to do at home.
- You ultimately decide how much care you will need!
Cash, check, credit card (except American Express) are accepted. A Flexible Spending Account (FSA) or Health Savings Account (HSA) can be used. Upon early request, receipts with treatment codes can be provided for you to seek out-of-network reimbursement from your insurance carrier.
What to Expect
Your initial appointment will be about 75 minutes and will include the following:
- Past and current health history
- Evaluation of pain/problems
- Discussion treatment goals
- Detailed postural evaluation/MYK treatment
Please arrive 15 minutes before your appointment time. If you haven’t already completed the paperwork there will be a clipboard in the waiting area for you. Subsequent visits are typically 30-55 minutes in length.
What to Bring
Rebalance Physical Therapy is fee-for-service and does not participate with insurance providers. This means patients make payments in full at the time of service. Although patients have insurance, they may find value in these exceptional benefits and save money too!
What to Bring
- Medical referral (not required, however, if you were given an MD order/referral, please bring it to your first visit)
- List of all current medications
- Water bottle
- Dress in loose fitting clothes to expose the problem area
- Please bring in all completed paperwork
Fees
Initial Evaluation and Treatment
75 minutes – $150
One-on-one evaluation and treatment. Evaluation includes thorough history taking, testing, and a discussion of the expected plan of care. Session includes manual therapy and exercise prescription. Clients will be educated regarding the cause of their pain or symptoms and ways to efficiently manage their symptoms.
Package deal 1:
five- 55 minute follow-up visits for $425 ($75 savings) *highly recommended
Package deal 2:
five- 30 minute follow-up visits for $315 ($35 savings)
As availability permits, special consideration and a reduced fee will be given to people in need.
Please note: Due to the complex nature of insurance, patients who choose the PT package may not be able to submit self claims.
Follow-up Visits
30 min – $70
55 min – $100
Session includes manual therapy and exercise prescription. Clients will be educated regarding the cause of their pain or symptoms and strategies to efficiently manage their symptoms.
